The coach approach empowers you to take leadership in your own health program. Research shows that you are more likely to reach your goals when: 1) you set them, 2) you decide how you'll reach them, and 3) you decide how you'd like to be held accountable to them. Your health and wellness coach will use evidence-based theories, methods, and techniques to generate the motivation and commitment to action that is necessary for sustainable change.

Application Interview (Discovery Consultation): This is your introductory call. Here we spend about 30-minutes getting acquainted and discussing the coaching agreement. This is a time for us to discuss your health and see whether we are a good fit for coaching. If we decide to move forward with your coaching program, you will then begin the intake process and receive your Baseline Assessment.
Session I (Baseline Assessment Review): We review your medical history and baseline assessment. This is also my opportunity to learn more about your lifestyle habits, current health condition, and daily routines. We will also begin discussing short and long-term health goals and continue the visioning process.
Session II (Personalized Lifestyle Medicine Program): This session expands on topics and themes covered in Session I and you determine specific health behavior changes for goal setting. We co-create SMART goals and agree on preferences for follow-up, communication, and accountability.
Sessions III (Design): This session is our final program design session. At the end of this session, you will have a clear health and wellness program to follow that aligns with your long-term goals. You may already have SMART goals you are working on at this time and can make changes to them as needed with your health coach.
Sessions IV-IIIV (Follow-up Sessions): These sessions are a support to your ongoing progress. It is my job to be a sounding board for you in these sessions and to use evidence-based behavior change methods to guide you into generative moments. These conversations provide you deeper reflection and greater insight into behavior motivations and answer the question of "how" to sustainably make lifestyle changes.
Maintenance and Ongoing Support: You have two options for ongoing maintenance after you end your 8 session coaching program.
